Doug McIlroy has told that story here, search the

It was indeed becase of the phototypesetter.
If I ever said that I must have been smoking something at the time.
The phototypesetter is a dramatic example, but a minor cause.
The major cause was pipes. Downstream programs fail
when they receive diagnostics rather than data. And users
never see the reason why.
